A regional health authority in Canada is seeking a Care Coordinator to manage assessments and care plans for clients in community and hospital settings. Candidates should have a Bachelor's Degree in a relevant field and at least 3 years of experience in a similar role. Responsibilities include authorizing services and working with multidisciplinary teams to ensure smooth discharges. The position offers hourly pay ranging from $46.46 to $54.22 and eligibility for a benefits package.
The Care Coordinator is responsible for assessment, care planning, authorization of services, and ongoing case management of clients referred to Continuing Care. The coordinator helps identify goals and formulates plans to meet identified client needs.
They authorize services, make referrals and linkages to other professional and volunteer resources, and work with interprofessional teams to facilitate hospital discharge to return to the community.
Care Coordinators work in community, hospital, or long‑term care facilities. They are fiscally responsible for the care, services, and equipment authorized within available policies and programs, maintain ongoing communication with clients, family, and health care teams, and manage reassessments at routine intervals.
We are looking for candidates who meet the following qualifications:
$46.46 – $54.22 hourly
