Job Details

Status: Contract full‑time until March 2026, 37.5 hours per week.

Salary: $61,201.01–$72,001.19 annually (starting salary based on candidate qualifications).

Location: 60 West Drive, Brampton, Ontario (opportunity for occasional remote work).

What You Will Do
  • Provide case management services to a caseload of clients.
  • Assess clients’ eligibility for supportive housing and facilitate placement in Housing and Support Peel units.
  • Establish rehabilitation goals and assist clients in reaching them.
  • Provide crisis intervention and link clients to appropriate programs.
  • Offer supportive counseling, life‑skills training, conflict resolution, budgeting assistance, and community integration.
  • Educate community agencies regarding mental‑health discharge planning services.
  • Collaborate with institutional staff to develop discharge‑planning policies.
  • Conduct case conferences with treatment providers and community organizations.
  • Maintain client records and documentation, and participate in team meetings and supervision.
  • Serve as a full member of the HASP team and provide court support as directed.
What You Need to Join Our Team
  • University degree or college diploma in Social Work or Social Services (BSW preferred).
  • Regulated Health Professional designation is an asset.
  • At least 3 years of relevant case management experience in a community mental‑health setting.
  • Experience working with clients in the criminal‑justice system.
  • Working knowledge of Ontario’s Forensic Mental Health System, Criminal Justice System, and recovery principles.
  • Ability to perform risk, need, and psychosocial assessments.
  • Strong communication skills in written and verbal formats with multidisciplinary professionals.
  • Knowledge of CRMS and OCAN (asset).
  • Training in ASIST, Non‑Violent Crisis Intervention and Crisis Response (asset).
  • Knowledge of French or another second language that reflects the diversity of Peel region (asset).
  • Current CPR/First Aid certification and valid driver’s licence with reliable, insured automobile.
  • Proof of full‑series Health Canada–approved COVID‑19 vaccination prior to start date (exemptions may be provided).
  • Satisfactory records search and vulnerable‑sector screening.
