Qualifications
- Minimum three years’ experience with an in-home/community care organization.
- Knowledge of London Middlesex Community Support Services providers is an asset.
- Valid Driver's License, insurance, and use of a vehicle required.
- Must acquire a current Police Vulnerable Sector Check.
Responsibilities
- Act as the first point of contact for client service eligibility.
- Participate in hospital rounds and discharge planning.
- Communicate with funders, clients, and care teams to coordinate services.
- Maintain statistical reporting of services as required.
