Qualifications

• Minimum three years’ experience with an in-home/community care organization.
• Knowledge of London Middlesex Community Support Services providers is an asset.
• Valid Driver's License, insurance, and use of a vehicle required.
• Must acquire a current Police Vulnerable Sector Check.

Responsibilities

• Act as the first point of contact for client service eligibility.
• Participate in hospital rounds and discharge planning.
• Communicate with funders, clients, and care teams to coordinate services.
• Maintain statistical reporting of services as required.
