Qualifikationen

• RN license (BCCNM) in good standing.
• Residence in Greater Victoria area.
• Valid driver’s license and reliable vehicle.
• Clinical nursing experience required.
• Experience in geriatrics, primary care, community nursing, long-term care, or chronic disease management is an asset.
• Strong communication and interpersonal skills.
• Ability to work independently while collaborating within a team.
• Commitment to patient-centered, relationship-based care.
• Full-time preferred; part-time considered.
• Background and reference checks required.

Aufgaben

• Conduct comprehensive patient histories and nursing assessments.
• Monitor and support patients with acute and chronic health conditions.
• Perform clinical procedures including wound care, injections, suture removal, and ear syringing.
• Administer vaccinations and prescribed medications.
• Monitor nutrition, medications, and overall health status.
• Perform screening assessments and preventive care interventions.
• Collect specimens and complete point-of-care testing.
• Provide education and guidance to patients and family caregivers.
• Collaborate with physicians and other members of the interdisciplinary team.
• Coordinate care with community resources, specialists, and hospital partners.
• Advocate for patients and help support aging safely at home.
