Job summary

ParaMed Home Health Care is seeking a motivated Occupational Therapist to provide community visits in Chatham Kent. The role involves assessing client needs, coordinating with a multidisciplinary team and equipment vendors to support functional independence.

Pediatric in-home or school services and adult health cases may be involved. You will deliver client-centered plans, collaborate with inter-professional teams, and maintain accurate records in line with ParaMed policies and regulatory

Qualifications

• Registration with the provincial College of Occupational Therapists in good standing.
• Accepting New Graduates licensed.
• ADP authorization considered an asset.
• Strong critical thinking and problem-solving skills.
• Knowledge of community resources available for clients and families.
• Current certification (within the past 12 months) of First Aid and CPR.
• A valid driver’s license and access to a reliable vehicle.
• Vulnerable Sector Screening (VSS) current.

Responsibilities

• Collaborate with a multi-disciplinary team of health workers and community resources.
• Analyze how physical, cultural, social and environmental factors affect occupational performance.
• Develop client-centered plans with clients and inter-professional teams, adapting services accordingly.
• Demonstrate evidence-based problem solving and judgement to address client needs.
• Engage clients with a collaborative approach to define services to be provided.
• Maintain up-to-date professional records in compliance with policies and regulatory standards.
• Maintain knowledge and skills to provide safe, effective service across practice areas.
