Qualifications

• Current immunization record including TB test.
• Valid driver’s license and vehicle for travel.
• CPR Certification.

Responsibilities

• Developing effective care plans in cooperation with Ontario Health.
• Providing nursing services both in-person and virtually.
• Communicating with interdisciplinary care teams.

Education
