Job summary

Jobtailor in New Westminster, BC is seeking a compassionate Licensed Practical Nurse (LPN) to provide in-home care and palliative support. You will follow the Care Designer's plan, administer medications, perform wound care, and monitor vitals to ensure safe, client-centered outcomes.

Comfort with mobile apps and documentation is required. The role emphasizes empathetic communication, collaboration with care teams, and helping clients with daily living activities while maintaining safety

Qualifications

• Negative TB Test.
• First Aid/CPR certification.
• Clear Criminal Record Check.
• Valid LPN license to practice in BC.
• Experience in hospital, home care or palliative care is an asset.
• Excellent written and verbal English skills.
• Other languages are an asset.
• Two professional references.
• Comfortable using mobile apps for care documentation.

Responsibilities

• Execute on the client’s care plan as outlined by the Care Designer.
• Provide delegations of task or other training to a care team.
• Assess client needs and report back to Care Designer with recommendations on complex care plans.
• Function within the limits of your experience and knowledge, while practicing safely and competently.
• Complete records and documentation accurately and thoroughly.
• Nursing tasks include medication administration, wound care, head to toe assessment, vitals, pain assessment, liaising with physicians, and diabetic management.
• Meet safety needs of the client and use equipment safely and properly.
• Assist clients with transfers, ambulation and exercises.
• Provide meal preparation, light housekeeping, companionship and personal care.
• Help clients with ADLs (Activities of Daily Living).
