Job description
Position Summary

The Community Liaison will play a frontline role in the Safe At Home project, working directly with individuals experiencing hoarding behaviours and serving as a bridge between tenants, service providers, and community supports. The position focuses on building trust, supporting housing stability, and reducing isolation through navigation, advocacy, and accompaniment. The Community Liaison will also strengthen relationships with partner agencies, municipal services, and natural supports, ensuring timely communication and coordinated responses to prevent eviction and enhance wellbeing. This role is ideal for someone with strong interpersonal skills, cultural humility, and experience working in mental health, housing, or community support settings.

Key Service Responsibilities And Specific Accountabilities
Direct Support to Tenants
  • Provide intake, initial engagement, and follow-up for tenants referred to the program.
  • Support tenants to access community resources, attend appointments, and participate in programs that enhance quality of life and housing stability.
  • Integrate Peer Support approaches to empower clients and reduce stigma.
  • Monitor progress and maintain regular communication with clients, ensuring early identification of challenges.
  • Apply trauma‑informed, harm reduction, and culturally safe practices in all interactions.
Community Navigation & Coordination
  • Serve as a connector between tenants, landlords, housing providers, and partner agencies.
  • Participate in case conferencing and coordinated intervention planning with the project team.
  • Support eviction prevention strategies by ensuring timely responses to tenancy concerns.
  • Help clients and their natural supports better understand available services and pathways to assistance.
Capacity Building & Engagement
  • Provide input into training development and delivery for community partners and allied professionals.
  • Contribute to advocacy, public awareness, and prevention efforts to address hoarding behaviours.
  • Represent the project in community forums, agency meetings, and stakeholder gatherings as assigned.
Data & Reporting
  • Maintain accurate case notes, service records, and client information in the project database.
  • Contribute to data collection and reporting for program evaluation.
  • Provide feedback to the Team Lead on trends, service gaps, and emerging needs.
Qualifications
  • Diploma or degree in Social Work, Human Services, Psychology, or related field (or equivalent combination of education and lived experience).
  • Minimum 3 years’ experience in mental health, housing support, or community‑based service delivery.
  • Strong knowledge of housing systems, community resources, and social service navigation.
  • Demonstrated skills in relationship building, advocacy, and conflict resolution.
  • Understanding of hoarding behaviours, harm reduction, and trauma‑informed practice (training can be provided).
  • Excellent communication skills, both written and verbal.
  • Competence with database entry, reporting, and Microsoft Office Suite.
Assets
  • Lived experience with hoarding, housing instability, or mental health/addiction recovery.
  • Experience working with diverse populations, including newcomers, Indigenous peoples, and seniors.
  • Training in motivational interviewing, crisis de‑escalation, or eviction prevention.
  • Ability to communicate in multiple languages.
Working Conditions
  • Community‑based role requiring regular travel to homes, agencies, and community locations.
  • Occasional evening or weekend work for community engagement or urgent client needs.
  • Valid driver’s license and reliable vehicle are considered assets.
