Job summary

Pacific Coast Community Resources is seeking a Community Inclusion Worker to assist individuals with disabilities in participating in personal, social, vocational, and volunteer activities within the community. You will support rights, coordinate activities, and document progress while maintaining high standards of care.

Requires Grade 12 education, a Community Support Worker Diploma or equivalent, First Aid/CPR and Food Safe certificates, driver’s license, and eligibility to work in Canada.

Qualifications

• Grade 12 education is required.
• Community Support Worker Diploma or equivalent is preferred.
• Current Emergency First Aid/CPR and Food Safe certificates are required.
• Satisfactory Criminal record clearance for children and vulnerable adults is required.
• Eligibility to work in Canada is required.

Responsibilities

• Ensure the supported person’s rights and choices are respected and advocated as needed.
• Interact effectively with people we support.
• Empower the person we support to be full citizens of their community.
• Coordinate, implement, and facilitate participation in community-based activities.
• Foster positive relationships with family, professionals, and community members.
• Support families in maintaining their home environment with basic household management skills.
• Participate in development and documentation of the Individual Service Plan.
• Record daily activity and PHI on the case management database.
• Apply behavior management principles and strategies as needed.

Education

Grade 12 education
