Job summary

Vancouver Island Health Authority in Duncan, BC is seeking a Community Health Worker to provide personal care assistance and support to clients and families to promote independence in the home. You will work within a multidisciplinary team to follow established plans of care and assist with daily living activities as needed.

Travel between sites may be required; a valid BC driver’s license is required. This role is part of a regular CHW team and involves caring for clients in the community

Qualifications

• Home Support/Resident Care Attendant/Health Care Assistant Certificate or equivalent
• Food Safe Certificate
• Valid BC driver’s license
• Current registration with BC Care Aide & Community Health Worker Registry

Responsibilities

• Provide personal care assistance and support to clients and their caregivers/families to promote and maximize independence in the home setting.
• Assist with activities of daily living and delegated tasks as directed and in accordance with plans of care.
• Travel between multiple sites may be required and a valid BC driver’s license is needed.

Job Description

As a member of a multidisciplinary health care team the Community Health Worker provides personal care assistance and support to clients and their caregivers/families to promote and maximize independence in the home setting, including assistance with activities of daily living and delegated tasks, as directed and in accordance with established plans of care.

Travel may be a requirement of this position. Transportation arrangements must meet the operational requirements of the Vancouver Island Health Authority in accordance with the service assignment and may require the use of a personal vehicle; and Valid BC Driver's License.

QUALIFICATIONS:

Education, Training And Experience

Home Support/Resident Care Attendant/Health Care Assistant Certificate from a recognized post-secondary program; or an equivalent combination of education, training and experience. Food Safe Certificate; and valid BC driver's license.

Current registration and in good standing with the BC Care Aide & Community Health Worker Registry.
