Job Summary

Personal Care Home Program

The Coordinator supports the Regional Personal Care Home Program by assisting Personal Care Homes to achieve compliance with Provincial Legislation and Operational Standards through assessment, education, evaluation and direction to Personal Care Home operators. The position is responsible for the coordination and conducting of annual reviews of Personal Care Homes. Reporting to the Regional Manager of the Personal Care Home Program, the Coordinator provides a leadership role within an interdisciplinary team through orientation and mentoring and providing direction with respect to interpretation of provincial standards. The Coordinator provides guidance to other team members with respect to responding to complaints regarding Personal Care Homes. The Coordinator, in consultation with the Regional Manager, plays a leadership role in developing and monitoring corrective action plans for Personal Care Homes that are non‑compliant with Provincial Operating Standards.

Placement Services

The CHN II is a clinical lead position with responsibility for providing clinical and administrative leadership to professional staff involved in the delivery of equitable and appropriate access to a broad range of long term care services in the Eastern Urban Zone. Services include access to: placement in nursing home, personal care home, short stay services and other models of care.

The CHN II liaises and consults with clients, families, members of the interdisciplinary team and other professionals in acute, long‑term care, rehabilitation, mental health and community‑based services. The CHN II assists in policy development, orientation, strategic planning and quality initiatives. This position plays a leadership role within Placement Services.

This position reports to the Regional Manager for Placement Services and maintains accountability to the Professional Practice Consultant for Community Health Nursing for professional integrity and practice issues.

  • Constantly required to sit at a computer or attend or facilitate meetings. Regularly stands to give presentations and occasionally drives between community nursing sites and personal care homes.
  • A higher than normal level of attentiveness and carefulness is required when reviewing clinical assessments and reports and for the health and safety of others. The high volume of paperwork received creates time pressures and requires a high level of concentration and alertness to detail.
  • Regularly subjected to time pressures and deadlines with an occasional lack of control over the pace of work, primarily when there are new procedures or products that require immediate education of Nurses.
Job Qualifications
  • Baccalaureate degree in nursing is required.
  • Current practicing license with the College of Registered Nurses of Newfoundland and Labrador (CRNNL).
  • A minimum of five (5) years nursing experience including at least two (2) years within the last five (5) years recent experience in the Personal Care Home Program is required.
  • Demonstrated knowledge of legislation, provincial standards and procedures relating to all aspects of the Personal Care Home Program.
  • Demonstrated leadership, analytical, problem solving, decision‑making and research skills.
  • Demonstrated ability to work collaboratively and with a high degree of independence and initiative.
  • Demonstrated time management skills and ability to prioritize.
  • Possess strong oral and written communication skills as demonstrated by delivering presentations, facilitating group exercises and writing reports.
  • Ability to establish and maintain effective working relationships. Possess knowledge of policies and procedures relating to all aspects of the Long‑Term Care Program.
  • A valid Newfoundland and Labrador Class 5 Driver's License and the use of a private vehicle are conditions of employment.
  • A satisfactory record of work performance and attendance is required.
