Job summary

Unison Health and Community Services in Toronto seeks Community Health Physicians to provide primary care to clients and families within a collaborative care model. The role emphasizes health promotion, client education, and episodic as well as chronic disease management in a community setting.

Two positions are available for a full-time, permanent schedule of 35 hours per week at the Bathurst-Finch site. Candidates must hold medical licensure, be in good standing with the College of Physicians

Qualifications

  • License to practice medicine and membership in good standing with the College of Physicians and Surgeons of Ontario.
  • CCFP Certification and CMPA Coverage.
  • Demonstrated experience in community-based, primary care settings.
  • Ability to work as part of an interdisciplinary team with clinical and program staff in a shared care model.
  • Excellent triage and clinical skills.
  • Experience with Electronic Health Records (experience in Practice Solutions Suite/PSS is an asset).
  • Ability to meet the expectation of providing episodic care, chronic disease management and trans care.
  • Commitment to pro-choice philosophy.
  • Familiarity with the social, economic and cultural conditions of the project’s geographic area, experience in working with local service providers, community groups and networks, and an understanding of local issues and priorities are an asset.
  • Experience serving Unison’s priority populations (seniors with complex needs, people with mental health & addiction issues,2SLGBTQ+, and youth facing barriers).
  • Demonstrated knowledge of, and sensitivity to, the impact of social, economic, environmental and cultural issues on diverse communities.
  • Knowledge of issues affecting communities facing barriers to access in an urban environment.
  • Effective communication (verbal and written), interpersonal, organizational and administrative skills, including computer literacy skills specific to job requirements.
  • Knowledge of a second (or more) language and culture is an asset.
  • Ability to work one evening a week.

Responsibilities

  • Provide primary care to clients and families within a multidisciplinary, collaborative care model emphasizing health promotion and client education.
  • Collaborate with team members to deliver episodic care, chronic disease management, and transgender-inclusive care as part of shared care.
  • Report to the Clinical and Allied Health Site Manager and participate in the Bathurst-Finch site operations.

Unison is seeking Community Health Physicians for ourBathurst- Finch site.
