Job summary
A regional health authority in Winnipeg is seeking a Community Health Facilitator to support the intake and assessment of clients in accordance with community standards. Qualifications
- Minimum of five years client service experience in community development, health or social services.
- Minimum of three years working in/with an interdisciplinary team.
- Valid Drivers’ License and access to a reliable vehicle required.
Responsibilities
- Facilitate the intake of new clients and assess program eligibility.
- Coordinate program activities with the team as directed.
