We are currently looking to fill a Casual opportunity for a Clinician, Hospice Palliative Care with Home Health located in Burnaby, B.C.

Detailed Overview

Coordinates, screens and prioritizes hospice palliative client activities by assessing clients, consulting with the interdisciplinary team and coordinating client care across the continuum of care. Acts as a clinical resource to other nurses and team members by providing clinical guidance and/or nursing consultation. Implements, evaluates and recommends clinical education plans for nurses and other staff in the designated clinical area. Provides nursing and clinical education/training, including training of nursing staff to deliver educational sessions. Reviews and evaluates care plans and takes corrective action.

Responsibilities

• Supports the interdisciplinary team by monitoring the standards of care and providing clinical nurse resources to staff; screens, assesses and prioritizes hospice palliative referrals; role models and mentors team members in therapeutic communication with clients, families and caregivers.
• Coordinates and prioritizes client care activities by providing formal clinical consultations to nurses, interdisciplinary team members and students; evaluates and modifies care plans to meet outcomes; discusses referral options with primary staff.
• Consults with the interdisciplinary team and other healthcare providers to resolve client care issues; supports critical thinking to address variances from guidelines; collaborates with clients and families to minimize variances.
• Develops and implements clinical education and orientation for the team; measures learning outcomes and applies appropriate teaching strategies.
• Assesses clinical skills, designs and evaluates education programs for hospice palliative care; provides training for new team members and ongoing education for staff.
• Acts as a preceptor and mentor; identifies skill gaps and provides instruction to ensure competencies align with quality patient care.
• Participates in quality improvement and risk management activities; supports client safety audits and policy improvement recommendations.
• Participates in rounds and care conferences; advocates for client rights and dignity.
• Participates in staff selection and performance evaluations; supports orientation and training needs and provides feedback to management.
• Promotes the use of research to support clinical practice and keeps program content current.
• Sets measurable goals and maintains clinical competence through professional development in collaboration with the Manager.
• Performs other related duties as assigned.

Qualifications

Education and Experience
Current practicing registration as a Registered Nurse and/or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three years of recent related clinical nursing experience in the designated clinical area, including one year in a supervisory/leadership role, or an equivalent combination of education, training and experience. Completion of applicable Canadian Nursing Association (CNA) certificate in the designated clinical area. Valid BC Driver's License and access to a personal vehicle for business-related purposes.
