Qualifications

Responsibilities

• Identify and engage eligible clients using screening techniques.
• Conduct social and psychosocial assessments of patients and families.
• Provide therapeutic counseling for clients experiencing illness-related circumstances.
• Facilitate clients' needs for recovery and promote autonomy.
• Follow up and monitor psychosocial situations.
• Act as a resource by providing information on community resources.
• Help clients explore reactions to loss and provide counseling.
• Coordinate with other service providers for client support.
• Participate in quality improvement activities.
• Educate health service providers about psychosocial aspects.
• Document clinical interventions and participate in assessments.
