Job summary

Kingston Health Sciences Centre seeks a clinical pharmacist to optimize individualized drug therapy within the HDH/KGH service programs. You will ensure safe procurement, storage, preparation, dispensing and administration of medications while engaging in education and research activities.

The role emphasizes promotion of patient- and family-centered engagement and care, and participation in hospital committees and quality initiatives.

Qualifications

Bachelor of Science in Pharmacy or PEBC-recognized degree.
Licensed as a Registered Pharmacist with the Ontario College of Pharmacists (OCP) within 12 months of starting.
Residency certificate preferred.
Minimum two years hospital pharmacy experience recommended.
Experience with drug information, pharmacokinetics and order entry.

Responsibilities

Function as a clinical pharmacist in assigned patient care service or program.
Identify, resolve and prevent drug-related problems with a patient-centered approach.
Provide drug information and therapeutic recommendations and intervene as needed.
Participate in interprofessional rounds and discharge management.
Report adverse drug reactions per hospital policy.
Perform medication histories, reconciliation and discharge counseling.
Conduct medication profile rounds and interpret serum drug levels.
