You will provide discharge counseling, immunization services, health screenings and medication reviews, while upholding Manitoba's scope of practice and professionalism.

Qualifications

• Licensed and in good standing with College of Pharmacists of Manitoba.
• Strong communication and interpersonal skills, written and verbal.
• Experience with immunization clinics, health screening, and community outreach.
• Discharge medication planning and clinical pharmacy services are valued.

Responsibilities

• Collaborate with physicians and healthcare professionals to optimize drug therapy outcomes.
• Conduct health screening clinics and immunization programs.
• Engage in community outreach initiatives.
• Perform medication reviews and medication management services per provincial standards.
• Apply full scope of practice as a clinical pharmacist in Manitoba.
• Counsel patients on non-prescription medications and pharmacotherapy.
• Ensure accurate pharmacotherapy and safe medication distribution.
• Advise patients and clinicians on indications, contraindications, interactions and dosages.
• Participate in medication preparation, packaging and distribution with attention to safety.
