• Communicates with physicians, patients, and health care team members for clarification and problem resolution.
• Provides medication counselling to patients and families and documents interactions in health records.
• Participates in obtaining medication history and assists with reconciliation.
• Maintains dispensing and inventory records and supports medication safety initiatives.
