Job summary

Avena Recruitment is seeking a Clinical Pharmacist for a Canadian healthcare provider serving seniors in long-term care, retirement, and community settings. You will collaborate with physicians, nurses, and other professionals to optimize therapy and improve patient outcomes.

You will conduct medication reviews, support quality initiatives, and maintain documentation in a patient-centered, interdisciplinary environment with a predictable Monday to Friday schedule.

Qualifications

• Licensed or eligible for licensure as a Pharmacist in Alberta.
• Strong clinical assessment and medication management skills.
• Excellent communication, interpersonal, and relationship-building abilities.
• Ability to work independently while collaborating within an interdisciplinary healthcare team.
• Strong organizational, planning, and time management skills.
• Previous experience in long-term care, retirement care, or clinical pharmacy is considered an asset.
• Valid Alberta driver's licence and willingness to travel throughout the Edmonton area.

Responsibilities

• Complete comprehensive medication reviews for residents in long-term care and retirement communities.
• Collaborate with physicians, nurses, and interdisciplinary healthcare teams to optimize medication therapy.
• Build and maintain strong relationships with long-term care homes, retirement communities, and healthcare partners.
• Conduct on-site clinical visits, medication audits, and quality improvement initiatives.
• Provide education to residents, families, and healthcare staff on medication management and clinical topics.
• Ensure compliance with provincial regulatory standards and professional practice requirements.
• Support clinical initiatives, pharmacy transitions, and ongoing quality improvement projects.
• Maintain accurate clinical documentation and patient records.

Position Summary
