Job summary

St. Joseph's Lifecare Centre Brantford is seeking a Clinical Manager to lead nursing teams, coordinate resident care, and ensure compliance with Long-Term Care regulations.

The role collaborates with the Director of Care and nursing leadership to maintain high-quality resident outcomes. The position is a Temporary Full-Time assignment through April 2028, reporting to the Director of Care, with responsibilities across resident care, quality improvement, workforce management, and safety.

Qualifications

  • Diploma in Practical Nursing and registration with the College of Nurses (Ontario).
  • Leadership training or experience in managing a workforce is required.
  • Recent long-term care experience at the nursing management level.

JOB SUMMARY
Purpose of the role

Reporting to the Director, Long-Term Care, the Clinical Managerworks collaboratively with the nursing leadership team to provide quality resident careconsistent with all applicable legislation including the Ministry of Long-Term Carerequirements, College of Nurses Standards, the Quality Inspection Process, RAI-MDS 2.0,St. Joseph’s Health System

KEY ACCOUNTABILITIES & SUCCESS CRITERIA
Resident Care
  • Supports the management of all aspects ofresident care in an efficient and effective manner, ensuring that resident’snursing, personal and safety needs are met by:
  • Daily review of residents’ records/riskmanagement reports in Point Click Care (PCC) meetings with the registerednurses to review high risk resident/family concerns and working with nurses toresolve, daily rounds on units
  • Participating in the development ofresident’s plan of care and ongoing review of resident plans of care foraccuracy, timeliness and resident specific focusses
  • Participating in admission/annual/palliativeand ad-hoc care conferences and assist with follow up on any action items
  • Monitoring of dining rooms to ensureresidents are receiving appropriate diets and staff are meeting compliance
  • Conducting case reviews with team onresidents with complex medical/nursing/risk or behavioural issues and followthrough on actions identified
  • Review/evaluating quality indicators such as,but not limited to: falls, wounds, responsive behaviours, restraints andresidents experiencing pain on assigned unit and ensure that appropriatemeasures and care plans are in place to manage
  • Providing direction, guidance and support toregistered staff and PSWs in the delivery of care and services
  • Ensuring surveillance of infections andtimely management of residents with infectious processes
  • Liaising between staff and communitypartners, allied health personnel, and consultants such as PsychogeriatricResource Consultant (PRC), Behavioural Supports Ontario (BSO)
  • Liaising with resident/SDM to provideupdates, gather information, assess, investigate and resolve resident/familyconcerns
  • Collaborating with the interdisciplinary teamin the management of resident care
  • Keeping the Director of Long-Term Careupdated on resident risk issues and work collaboratively to address/resolve
Quality, Acts and Regulations
  • Ensure staff and the home is in compliancewith all applicable legislation/standards including the Fixing Long Term CareAct and regulations;
  • Have thorough knowledge of the Fixing LongTerm Care Act and Regulations
  • Participate in the review Long Term Careapplications for admission
  • Continuously develop, implement and evaluatequality improvement/best practice initiatives and participate in ContinuousQuality Improvement Activities
  • Analyze and evaluate resident care andservices using the inspection protocols developing action plans and workingwith staff to implement
  • Provide education to staff on the act andregulations at unit team meetings
  • Act as the program lead for required clinicalprograms as assigned
  • Ensure adherence by staff to all requiredMinistry of LTC requirements and Clinical programs
  • Participate in the annual required programevaluations
  • Monitor staff’s timely completion of RAI MDSand care plans and follow up accordingly
  • Complete Critical Incident Reports as perlegislation and follow through on any required follow up action
Workforce Management
  • Leading a team of qualified and competentstaff, ensures accountability for a high standard of nursing and personal careservice and high levels of customer service by:
  • Acting as a role model and contributing tothe development of a caring and competent team who is responsible andaccountable for direct resident care activities.
  • Facilitating teamwork and collaboration amongthe team
  • Having thorough knowledge of policies andprocedures
  • Having thorough knowledge and understandingof the Collective Agreements
  • Reviewing new or revised policies andprocedures with teams
  • Reviewing customer service principles withstaff at team meetings
  • Coaching, mentoring and providing feedback onperformance of staff on an ongoing basis.
  • Reviewing staff documentation and nursingcare activities to ensure they are in compliance with College of NursesStandards and facility policies
  • Identifying performance gaps and workingcollaboratively with staff involved and other resources in the development ofperformance improvement/management plans
  • Completing staff performance appraisals in atimely manner including the management of probationary employees
  • Monitoring and managing staff availability,attendance and tardiness concerns
  • Maintaining an open-door policy and follow upon staff concerns
  • Assisting with all aspects of therecruitment, selection and retention of staff, including functions such aspreparing postings, granting positions, completing necessary paperwork fortransfers/granting of positions
Safety and Security
  • Ensurea safe working environment for staff and living environment for residents
  • Conductdaily rounds on units including service area, noting and addressing anyconcerns
  • Participatesin Health and Safety activities such as inspections and as a member of theHome’s Joint Health and Safety Committee
  • Maintainsa safe working environment by adhering to proper procedures and promptreporting of unsafe conditions
  • Participateand coach staff through emergency codes and drills such as, but not limited to:fire/evacuation/missing resident
  • Respondsto and participates in emergency situations and emergency preparednessexercises as required
  • Ensurestaff’s compliance with Occupational Health and Safety Policies and Ministry ofLabour legislation.
  • Ensurestaff are completing routine equipment inspections as scheduled, i.e.lifts/slings
  • Ensuresthat staff incidents/accidents are reviewed and properly investigated inaccordance with Incident Reporting Procedure;
  • Participatein the safe and early return to work program, ensuring that there is monitoringand follow up with staff for the duration of the program
General
  • Performs other specific nursing programs,interdisciplinary committee work, projects and duties as assigned
  • Ensures strong fiscal accountability andcontributes to the administration of the resident care budget
  • Maintains effective communication with allmembers of the Home Management Team through email, reports, management teammeetings and other discussions
  • Makes recommendations for changes to increaseservice, improve efficiency and/or make recommendations for the effectiveutilization of resources
  • Attends external educational opportunitiesand share outcomes
  • Covers the home in the absence of theDirector Long Term Care and/or Assistant Director of Care
  • Performs other delegated duties as assigned
Health and Safety
  • Works in compliance with OHSA and itsregulations and the rules and procedures of the Health and Safety program
  • Maintains a safe working environment byadhering to proper procedures
  • Reports health and safety hazards,malfunctioning equipment or missing protective devices
  • Participates and follows infection controlpolicies and procedures
  • Takes an active role in protecting andpromoting their own health and safety and that of clients, caregivers and otherteam members
  • Reports any illnesses or injuries to theirsupervisor immediately
  • Wears personal protective equipment asdesignated.
QUALIFICATIONS & SKILLS
a) Formal Education
  • Diploma in Practical Nursing and registeredwith the College of Nurses
b) Other Knowledge,Skill, and Experience
  • RN or RPN registration with a currentcertificate of competence from the College of Nurses of Ontario;
  • Successful completion of a Practical NursingProgram or Bachelor of Nursing degree or working towards, from a recognizededucational institution
  • Leadership Education or Training withdemonstrated effectiveness in managing a workforce
  • Recent Long Term Care experience at thenursing management level with the ability to effectively manage human resourcesand day to day labour relations
  • Sound administrative ability, good knowledgeof the principles and practices of nursing management is required
  • Working knowledge of computers, theirapplication and related software i.e. Microsoft Office, Point Click Care,MDS-RAI,
  • Possess knowledge of the legislation in LongTerm Care, as well as other government legislation in the areas of labour,employment, health and safety and human rights
  • Highly effective communication skills bothverbal and written with strong interpersonal and organizational skills and arealso required
  • Strong problem solving, teamfacilitation/development and conflict resolution skills
  • Experience in leading, supervising,motivating unionized and non-unionized staff to achieve desired results.
