Requirements / Qualifications
  • A regulated health professional RN / RPN with a minimum 2 years experience
  • Preferably 2-3 years recent experience with progressive responsibilities as a manager in the community setting.
  • Solid knowledge of principles, practices, and methods of community nursing and service delivery to clients.
  • Able to supervise a team of Personal Support Workers.
  • Valid drivers license
  • Strong time management skills, confidentiality and professionalism
  • Excellent communicator, able to foster relationships with Home Care Staff and clients
  • Ability to work independently and work effectively within a team environment
  • Interest in continuous learning – high energy and focused
  • Current CPR-C / BLScertification
  • Police Criminal Record Check with Vulnerable Sector Screening.
Responsibilities
  • Compassion, integrity, and quality care are all expectations of our community culture.
  • Assess or arrange for the initial and ongoing assessments of clients requiring unlicensed care and service as appropriate.
  • Lead and direct a PSW team to deliver service experience that meets clients needs and expectations. This includes supervising, coaching and supporting the Personal Support Workers to provide high client focused service that complies with standards of practice as well as company policies.
  • Develop and / or ensure that care plans are available for clients as appropriate; visit clients as required and update care plans as appropriate.
  • Conduct training sessions as well as facilitate and monitor training and educational opportunities for Personal Support Workers.
  • Ensure that all clients concerns and complaints are addressed and documented in a timely manner.
  • Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating body.
  • Communicate and work closely with Coordinators responsible for scheduling Personal Support Workers
  • Work with recruitment in the selection, orientation and training as well as the discipline and annual performance of Personal support workers.
  • Maintain confidentiality of client, PSW and corporate information and discuss only with appropriate staff.
  • Participate in proactive Health & Safety activities while performing all duties. Notify immediate Supervisor of any Health Concerns.
  • Complete all other tasks as required.
