Reporting to the Director, Kitikmeot Regional Health Centre, the Client Clinical Care Coordinator is responsible for the coordination of clinical care services for the clients in their assigned region who are accessing medical travel and specialist care programs both within and out of Nunavut. The primary goal of this position is to support client and family well-being while they are progressing through multiple systems of care.

The Client Clinical Care Coordinator plays an important role in the health system by proactively identifying and working with clients with complex chronic conditions, to provide coordination and navigation of care and support across health and care services. Remote communities face barriers to accessing high quality, coordinated care. The Coordinator creates opportunities by working closely with the interdisciplinary care team (including case managers, discharge planners, physicians, nurses, social workers) to reduce these barrier, make healthcare more accessible, and ensure appropriate care is provided as close to home as possible, including accessing vital care services.

The knowledge, skills, and abilities required for this job are typically acquired through an undergraduate degree in nursing, social work or related field and two years of health, social service or related work experience within a clinical environment.

This is a Highly Sensitive Position and a satisfactory Criminal Record Check, along with a clear Vulnerable Sector Check is required.

The Official Languages of Nunavut are Inuktut, English and French. Applicants may submit their resume in any of the official languages of Nunavut. Fluency in more than one of Nunavut’s official languages would be considered an asset. Knowledge of Inuit communities, culture, land,Inuit Qaujimajatuqangit, Inuktut and experience working in a northern cross-cultural environment are also considered assets.
