Access Healthcare Services Inc. is looking for a dedicated Manager, Client Services Coordination with a passion for home and community care to join our growing team and support the Access Healthcare team.
As a Client Care Supervisor, you will provide, coordinate, and promote high-quality, client-centered care to individuals receiving home and community-based services, including those funded through Ontario Health atHome programs. Working within the scope of practice of a Registered Practical Nurse (RPN), responsibilities include conducting client assessments, providing PSW delegation and training, developing and evaluating care plans, and supporting client education and health promotion.
The Client Care Supervisor provides clinical supervision and support to Personal Support Workers and Home Support Workers delivering care in clients' homes and collaborates with clients, caregivers, Ontario Health atHome care coordinators, and the interdisciplinary team to ensure safe, effective, and responsive service delivery.
The role requires strong clinical judgment, customer service skills, and a commitment to quality documentation, regulatory compliance, and continuous quality improvement.
