Job summary

Access Healthcare Services Inc. seeks a Client Care Supervisor (RPN) to lead and coordinate home and community-based care in Ottawa.

You will supervise Personal Support Workers, delegate tasks, and ensure safe, client-centred plans within the Ontario Health at Home framework. Holders of an accredited LPN program with current CNO registration and at least 12 months in home health will thrive in this hybrid role, collaborating with a wide team to maintain high-quality service and regulatory

Qualifications

  • Graduation from an accredited Licensed/Registered Practical Nursing Program.
  • Current registration with the College of Nurses of Ontario (CNO).
  • At least 12 months' experience in home or community health care or related roles.
  • Experience handling HR aspects and interpreting company policies.
  • Proficient computer skills including Microsoft Office and virtual meeting platforms.

Responsibilities

  • Conduct comprehensive client assessments and reassessments.
  • Develop and evaluate individualized, client-centered care plans.

Client Care Supervisor (RPN), Personal Support Services

Personal Support Services Full Time Ottawa, ON Hybrid $29 - $36.50 CAD per hour

Access Healthcare Services Inc. is looking for a dedicated Manager, Client Services Coordination with a passion for home and community care to join our growing team and support the Access Healthcare team.

POSITION PROFILE

As a Client Care Supervisor, you will provide, coordinate, and promote high-quality, client-centered care to individuals receiving home and community-based services, including those funded through Ontario Health atHome programs. Working within the scope of practice of a Registered Practical Nurse (RPN), responsibilities include conducting client assessments, providing PSW delegation and training, developing and evaluating care plans, and supporting client education and health promotion.

The Client Care Supervisor provides clinical supervision and support to Personal Support Workers and Home Support Workers delivering care in clients' homes and collaborates with clients, caregivers, Ontario Health atHome care coordinators, and the interdisciplinary team to ensure safe, effective, and responsive service delivery.

The role requires strong clinical judgment, customer service skills, and a commitment to quality documentation, regulatory compliance, and continuous quality improvement.

ROLES & RESPONSIBILTIES
  • Conduct comprehensive client assessments and reassessments to support positive clinical and client outcomes.
  • Develop, implement, and evaluate individualized, client-centered care plans in collaboration with clients, families, and the interdisciplinary care team.
  • Provide clinical supervision, mentorship, delegation, and ongoing support to Personal Support Workers and Home Support Workers delivering care in clients' homes.
  • Monitor service delivery through client visits, care reviews, and quality assurance activities to ensure safe, effective, and responsive care.
  • Collaborate with Ontario Health atHome care coordinators, funders, caregivers, and other healthcare professionals to coordinate services and achieve optimal outcomes.
  • Participate in quality improvement initiatives and support the organization's quality management and risk management programs.
  • Promote evidence-informed practice through staff education, coaching, and knowledge sharing.
  • Address client concerns and complaints in a timely manner, supporting a positive client and family experience.
  • Contribute to recruitment, onboarding, performance management, and professional development of direct reports.
  • Maintain accurate and timely documentation, ensure regulatory compliance, and support the effective use of electronic health records.
  • Participate in the management on-call rotation to provide operational guidance and escalation support as required.
  • Engage in ongoing professional development and perform other related duties as assigned.
EDUCATION AND TRAINING
  • Graduation from an accredited Licensed/Registered Practical Nursing Program;
  • Current registration with the College of Nurses on Ontario (CNO)
EXPERIENCE
  • At least 12 months' experience with progressive responsibilities, preferably in a home or community health role or experience working in home and community care.
  • Demonstrated ability to handle all aspects of human resources and interpret company policies and procedures;
  • Proven computer technology skills e.g. Microsoft applications, and ability to use virtual meeting forums.
  • Sound understanding of the principles, practices, and methods of community nursing and service delivery
REQUIRED SKILLS
  • Exceptional interpersonal skills and ability to handle difficult situations objectively and consistently;
  • Ability to work independently and effectively lead a high performing team;
  • Demonstrated team-building and conflict resolution skills;
  • Excellent writing, reporting and English communication skills;
  • French language proficiency highly desirable
  • Valid driver's license and auto insurance
  • Evidence of current CPR certification; and clear criminal background check including vulnerable sector screening
COMPENSATION
  • $29.00 - $36.50 per hour (commensurate with experience)
