Responsibilities

• Assign appropriate staff to patients based on factors such as patient needs, staff availability and skill sets.
• Build a care schedule for patients.
• Enter and update employee and patient data within the computerized system.
• Respond professionally to calls and electronic communication from funders, staff, patients and families.
• Document and report all staff and patient interactions.
