Job summary

CMHA Durham is seeking a dedicated Case Manager for the HART Hub in Oshawa to provide client-centered support services to mental health and addictions clients.

You will work within an integrated care model, completing intake assessments, developing individualized care plans, and coordinating services to promote recovery and independence. Strong communication, collaboration, and organizational skills are essential.

Qualifications

  • Post-secondary education in related field and minimum one year related experience or equivalent

Responsibilities

  • Participates as a member of a high-performing multi-service team supporting clients in Addiction and Mental Health services
  • Provides service in an integrated shared care model
  • Processes client intakes and reviews assessment information; identifies goals and prepares individualized care plans
  • Develops and revises treatment goal plans with clients and team
  • Explains CMHA Durham services and ensures client consent to Hart Hub processes
  • Supports clients in one-on-one and group settings to promote housing stability and healthier living
  • Delivers respectful, individualized case management and supports independent living
  • Assists with appointments, transport, and coordination of care
  • Encourages peer supports and social connections for clients
  • Establishes therapeutic relationships and advocates for clients
  • Collaborates with community resources to ensure continuity of care
  • Documents client contacts and prepares reports per CMHA Durham policies
  • Performs other tasks as assigned by management
  • Maintains client safety and adheres to infection control procedures

Job description

Who We Are:

Canadian Mental Health Association Durham is an integrated community mental health and primary care hub that has been providing mental health services to Durham Region for over 60 years.

During that time, it has evolved to provide a broad range of services to individuals and their families experiencing mental health challenges, from information and education to case management, psychosocial rehabilitation, housing and primary healthcare. These services are provided through inter-professional teams focused on providing clinically based services that, in addition to dealing with client mental health issues, address social determinants of health using the recovery model.

Overview of the Role:

Reporting to the HART Hub Lead, this position provides case management, advocacy and support services to mental health and addictions clients within the HART Hub of the Canadian Mental Health Association (CMHA) Durham. They will also play a role in working with clients and as a member of a multi-disciplinary team to identify and prioritize goals that foster recovery and Independence using a client-directed case management approach.

Location: HART Hub employees may begin work at 60 Bond St. West, and will move to the designated HART Hub location in Oshawa at a later date.

Duties and Responsibilities:
  • Participates as a member of a high performance, multi-service team supporting clients living and participating in programs for Addiction and Mental Health supports at the HH
  • Provides service in an integrated shared care model within a team
  • Processes client intakes and reviews assessment information by identifying goals and preparing individualized supportive client care plans that promote long- term stability and growth
  • Develop, implement, evaluate and revise treatment goal plans in collaboration with clients and team members
  • Ensures that clients understand CMHA Durham services and agree to the expectations related to Hart Hub services using appropriate service agreements
  • Works with clients in one-on-one and/or group situations teaching life skills that promote sharing of responsibilities associated with reaching their goals related to living a healthier life through stabilization, recovery and appropriate housing
  • Provides case management services in a respectful individualized, supportive and flexible manner
  • Supports and assists clients with developing their skills to live independently in the community and promotes safe and healthy living standards ensuring that client’s basic needs of food, clothing and shelter are met
  • Provides support services to clients assisting with scheduled appointments, arranging transportation to appointments and/or accompanying clients to appointments, as necessary
  • Supports clients to establish and foster peer supports and meaningful social connections
  • Establishes supportive therapeutic relationship with clients and advocates for, and with, clients
  • Consults and collaborates with community resources and service providers ensuring clients obtain the most appropriate services and receive continuity in their care
  • Documents client contacts, creates daily logs, summaries and completes reports in a timely manner, as outlined in the CMHA Durham policies and procedures
  • Performs other tasks as assigned by management
Client Safety:

Adhere to CMHA Durham client safety programs including program policies as well as infection control and prevention procedures. In partnership with clients, family members, and fellow staff, maintain open lines of communication to assist the organization in identifying and achieving quality outcomes for client safety. Report client safety incidents and suggested improvements to manager or delegate.

Other Duties as Assigned:
  • Attend, participate in team meetings and all employee meetings
  • Participate in organizational quality improvement activities, e.g. accreditation projects, committee work, special events
  • Participate in quality improvement activities at the team/program level

Minimizes risks adhering to CMHA Durham Policies and Procedures identifying, assessing, evaluating and controlling risks ensuring well-being of employees and clients

Qualifications:
  • Post-secondary education in a related area of study (i.e. mental health, social sciences or human services) and a minimum of one-year related experience or equivalent combination of education and experience
  • Sound knowledge of the Mental Health Act, Health Care Consent Act, social service benefits and related legislation in Ontario
  • Ability to develop and maintain a positive therapeutic relationship with clients
  • Ability to develop and maintain positive relationships with community partners and represent the agency in a professional
  • Ability to acquire and utilize community resources effectively to meet client needs
  • Excellent communication (written and verbal) skills
  • Ability to work with individuals of diverse backgrounds
  • Ability to organize and present information
  • Flexibility to change
  • Ability to work independently when necessary
  • Ability to think critically and creatively
  • Valid Ontario Driver’s license, access to a dependable vehicle in safe working order and appropriate insurance coverage
  • Works a variable work schedule, which may include evenings and weekends, as required
  • Knowledge of MS Office 365 (Word, Excel, PowerPoint, Access and Outlook), as well as the internet
  • Knowledge of Indigenous communities and cultural practices (preferred).
  • Intermediate fluency in French considered an asset
  • Work outside of normal business hours as required, in compliance with the Disconnect from Work policy.
