A leading mental health agency in Toronto is seeking a Case Manager for the East Assertive Community Treatment Team. Responsibilities include conducting assessments, developing service plans, and providing therapeutic support to clients with mental health challenges. The ideal candidate will have a post-secondary degree in social services and a commitment to recovery-oriented principles. This full-time permanent role offers a salary ranging from $54,943 to $63,988.
The Canadian Mental Health Association (CMHA) – Toronto is looking for a Case Manager for our East Assertive Community Treatment Teams (ACTT). Based at the Markham site, the multidisciplinary ACT Team provides community-based support, rehabilitation, and treatment to individuals living with mental illness. The team focuses on promoting stabilization, community integration, recovery, and quality of life through client‑centered services delivered in the community.
