Job Description

Case management social worker to work in a community service organization, supporting individuals with intellectual disability. Duties include:

  • Interview clients individually, in families, or in groups to assess their situation and determine required services.
  • Plan programs of assistance for clients.
  • Refer clients to agencies providing financial assistance, legal aid, housing, medical treatment and other services.
  • Serve as a member of an interdisciplinary team of professionals working with a client group.
Qualifications
  • Bachelor’s degree or College/CEGEP in social work or related field.
  • 1 year to less than 2 years of work experience in case management or community development.
  • Valid driver’s licence and own vehicle for transportation.
  • Vulnerable sector check required.
