Qualifications
- Registered with the College of Nurses of Ontario or equivalent.
- At least 2 years of related professional experience.

Responsibilities
- Assess referred clients for admission and establish helping relationships.
- Perform multi-dimensional assessments and identify needs.
- Establish care and service plans balancing client needs and resources.
- Initiate and authorize implementation of care plans.
- Conduct discharge processes and redirect clients as necessary.
