Qualifications

• Registered with the College of Nurses of Ontario or equivalent.
• At least 2 years of related professional experience.

Responsibilities

• Assess referred clients for admission and establish helping relationships.
• Perform multi-dimensional assessments and identify needs.
• Establish care and service plans balancing client needs and resources.
• Initiate and authorize implementation of care plans.
• Conduct discharge processes and redirect clients as necessary.
