A governmental organization in Whitehorse is seeking a dedicated Social Worker to join their team. This role requires current registration with a Canadian regulatory body, relevant case management experience, and the ability to create and implement person-centred care plans. The ideal candidate will work closely with a multidisciplinary team to ensure holistic care tailored to residents' needs and values. This posting will be used to fill one (1) casual full-time position working 75 hours bi-weekly, ending March 17, 2026. It will establish a 12-month eligibility list which may be used to fill anticipatory permanent, term, AOC and casual vacancies.

Are you a Social Worker with current registration who is eager to actively engage with residents, their families and their care teams to understand and capture their goals, values, and preferences and incorporate these elements into their daily care routines? As a key member of the multidisciplinary team, you will be informing and creating care plans and supporting continuity of care, focused on meeting physical and medical care needs but also supporting emotional, psychological, and social well-being. As part of this dynamic approach, you’ll conduct thorough assessments and work with the multidisciplinary team in developing targeted interventions, within your scope of practice, to ensure interventions are tailored and effective, while providing continuous support during transitions into and throughout an individual’s journey through care.
This role provides compassionate, holistic, and individualized care that prioritizes the residents' needs and values, ensuring their dignity and autonomy are respected. This kind of approach to care emphasizes the partnership between healthcare providers and residents, where the residents' perspectives directly inform the care planning process.
