As a Care Coordinator (Status: Full-Time, Start date: Immediately;Location: North Bay), you will assess and determine patient care needs andeligibility, provide access and referrals to community services, and engagewith patients, caregivers and other health care practitioners.

What will you do?

  • In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans

  • Establish a helping relationship with patients and their families
  • Balance patient needs and choices with available resources,ensuring patients’ values and preferences are respected

What must youhave?

  • OntarioCollege of Social Workers and Social Service Workers
  • Recentexperience in community health or a related field
  • Knowledgeof the health care delivery system and community resources
  • Excellentinterpersonal, communication, assessment, problem-solving anddecision-making skills
  • Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busymultidisciplinary environment
  • Establishedability to accurately complete required documentation, reports and forms
  • Avalid driver’s license and access to a reliable vehicle
  • Advancedoral and written proficiency in English is essential
  • We have amandatory COVID-19 vaccination policy. As a condition of employment, allemployees are required to submit proof of COVID-19 vaccination statusprior to start date.

What would give you the edge?

  • Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics
  • Casemanagement experience or recent related community experience
  • Abilityto speak French or another second language

Salary: Hourly wage of $41.29 to $51.13
