We are seeking a dedicated and compassionate Care Coordinator (Case Management) Liaison to join our team in supporting individuals experiencing homelessness. This role is responsible for providing direct case management and supportive services to emergency shelter clients, while serving as a key connection between clients and the Trenton/Mercer Continuum of Care (CoC) Coordinated Entry and Assessment System (CEASe).
Located in Trenton, New Jersey we serve men and women who have no place else to turn for shelter, clothing, treatment, and food. We provide a safe, clean, warm refuge for the homeless, the hungry, the transient and the addicted. Our services encourage individuals to reach their greatest potential and create an environment where one can live with dignity, build self-esteem, responsibility and hope. Rescue Mission of Trenton was founded in 1915 and is a non-profit organization that helps rescue people from themselves and their situations. Our dedicated staff in the Shelter and our Behavioral Health Unit offer care, support and encouragement to all that reside here.
We value dignity, communication and helping each of our clients reach their full potential. Our clients are assisted by staff with treatment options, medical help, job placement, financial planning, education, case management and mental health treatment. This position is an important part of helping our clients navigate independent living.
