Your role as Care Coordinator

Dialogue is seeking a Care Coordinator to join our team on a temporary contract to enhance the well‑being of our members and support the administrative needs of our healthcare team. In this hybrid role, you will focus on essential tasks such as appointment scheduling, prescription processing, and general administrative support.

What you’ll be doing
  • Act as the first point of contact between patients and the medical team.
  • Assist patients with scheduling appointments and follow up after appointments, including sending prescriptions, scheduling follow‑up appointments, sharing navigation resources, and sending recommendations by fax.
  • Act as a liaison between medical and technical support teams to help patients and providers resolve various medical, technical, or logistical issues.
  • Assist patients in finding resources to deliver a memorable experience.
  • Be committed to ensuring total customer satisfaction.
  • Communicate with pharmacies and other healthcare facilities by fax or phone, as needed.
  • Provide administrative assistance to clinicians.
  • Process incoming documents by fax and mail.
  • Advise patients to help them navigate their province's medical system.
  • Ensure efficiency throughout the patient journey in collaboration with members of the healthcare team.
