What You'll Do

Be the First Face of Kindred Care

  • Visit new clients in their homes to assess needs, conduct safety evaluations, and build tailored care plans that reflect their goals and values.
  • Collaborate with internal teams to ensure care starts smoothly, on time, and aligned with the client’s preferences.

Support Ongoing Client Success

  • Conduct reassessments for existing clients, adapting care plans as needs evolve.
  • Flag risks, challenges, or opportunities for enhanced care—and work with your team to find solutions that work.

Be a Trusted Care Partner

  • Assist with occasional operational tasks or follow-up visits that ensure clients are set up for success—based on evolving care needs.
  • Blend this role into your weekly care schedule, adding variety and leadership to your existing work.
What You Bring
  • PSW certification, Internationally Educated Nurse (IEN), or are a Retired RN or LPN.
  • 3+ years of demonstrated experience in healthcare.
  • Your own vehicle (and a reliable GPS helps too!)
  • A people-first mindset and ability to quickly build trust.
  • Strong written and oral communication skills.
  • Curiosity, clarity, and compassion in every client interaction.
