Qualifications

  • Membership, in good standing, with the College of Nurses of Ontario.
  • Recent experience in community health or a related field.
  • Knowledge of health care delivery system and community resources.
  • Excellent interpersonal and communication skills.
  • Effective time management and organizational skills.
  • Valid driver’s licence and access to a reliable vehicle.
  • Advance proficiency in oral and written English and French.

Responsibilities

  • Assess care needs and determine eligibility for services.
  • Link patients with service providers.
  • Coordinate and monitor care plan delivery.
  • Establish helping relationships with patients and families.
