What You’ll Be Doing / Ce que vous ferez

As a key member of our care team, you\'ll:

  • Conduct In-Home Health Assessments
  • Perform initial and reassessments in client homes
  • Evaluate physical, mental, and environmental health conditions
  • Identify safety concerns and barriers to care
  • Maintain and Update Client Records
  • Document care plans, assessments, and client progress accurately and in a timely manner
  • Ensure full compliance with healthcare privacy and documentation regulations
  • Develop Individualized Care Plans
  • Create thoughtful, person-centered care plans and client biographies
  • Work closely with social workers, families, and other health professionals to implement the best plan of care
  • Analyze Data and Drive Improvement
  • Interpret client data to identify trends, risks, and opportunities
  • Recommend process improvements that support quality, safety, and efficiency
What You Bring to the Role / Ce que vous apportez au poste
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) with valid New Brunswick registration
  • 2+ years of relevant professional healthcare experience (homecare, disability, or case management)
  • Bilingualism (English & French - both written and spoken)
  • Passion for community care and improving lives in meaningful ways
  • Proficiency with documentation software and a strong grasp of medical terminology
  • A valid New Brunswick driver’s license, clean driving abstract, and daily access to a reliable vehicle
  • Clean criminal record check and Social Development Background Check
