Job Summary

The Behaviour Personal Support Worker, Behavioural Support Services Mobile Support Team (BSS-MST), is part of a team that provides assessment, strategy development, education and support to older adults who are demonstrating responsive behaviours that may be associated with various types of dementia, mental health, addictions or other neurological conditions who live in various community settings within York Region / South Simcoe. Working with a team of other professionals, the BPSW will enhance the quality of life for clients with responsive behaviours in community settings by developing and implementing person‑centered strategies to mitigate responsive behaviours. This position will also assist in the assessment and development of a Behavioural Support Plan. In addition, the BPSW will assist and provide training for clients, families and other ongoing care partners.

What You Will Do
  • Act as the Primary PSW for assigned person(s) served and follow the weekly schedule of required visits.
  • Perform and record a complete and accurate observation of the person served and their responsive behaviour during the initial 2‑week period after enrollment and assist in identifying triggers for the responsive behaviour.
  • Document activity according to agency standards in the client information system and report observational data using specified tools (BSO Questionnaire, etc.).
  • Offer suggestions to the development of an individual Behavioural Support Plan with emphasis on strategies to manage responsive behaviours.
  • Actively practice the suggested interventions and continually adapt them to ensure the best possible outcome for mitigating the responsive behaviours.
  • Accept direction from the Psychogeriatric Case Manager regarding the Behavioural Support Plan and scheduling of visits to the person served.
  • Model effective interventions/strategies for ongoing care partners, caregivers and/or family members using peer‑to‑peer mentoring.
  • Attend Behavioural Support Plan and discharge meetings with ongoing care partners and families.
  • Participate in multi‑disciplinary conferences, care conferences for the person served, and staff development conferences.
  • Actively assist and support the transition of persons served to their next care destination.
  • Work in compliance with the Health and Safety Act and its regulations in performing duties safely and following all health and safety policies, procedures and legislation.
  • Follow standard infection control practices and enhanced precautions as instructed.
  • Complete other duties, as assigned by the Associate Director and/or Program Manager.
What You Bring
Must Haves
  • Completion of post‑secondary education in Human Services (e.g., Personal Support Worker Certificate, Health Care Aide Certificate, Registered Practical Nurse Certificate).
  • Experience with vulnerable, older adults and seniors with serious mental illness, cognitive and addiction challenges who exhibit responsive behaviours.
  • Knowledge of GPA, P.I.E.C.E.S., U‑First or willingness to receive such training.
  • Sound understanding of and commitment to the principles of the Behavioural Supports Ontario Project.
  • Knowledge and proven experience working with clients in a PSR (Psychosocial Rehabilitation) approach.
  • Ability to organize and work with individuals and groups in the areas of assessment, problem‑solving and crisis management.
  • Ability to interact with clients in a positive and supportive manner.
  • Physically able to perform demanding body mechanics (e.g., lifting, transferring).
  • First Aid and CPR certification.
  • Good oral and written skills; excellent computer skills; ability to follow written and verbal instructions.
  • Ability to work effectively in a team environment.
  • Demonstrated effectiveness working with minimal supervision, proven reliability and trustworthiness.
  • Valid driver’s license and access to own vehicle.
