Pillar Health, A Division of KCHC is seeking a Behavioral Health and Resource Specialist.
POSITION SUMMARY
The Behavioral Health and Resource Specialist (BHRS) provides professional, non-clinical support to patients and care teams across Behavioral Health, Psychiatry, Medical, Dental and enabling services. The Specialist exercises independent judgment to support access to care, coordinate referrals, resolve social needs, and improve continuity of care through integrated models. The Specialist will primarily focus on Behavioral Health intake, navigation and care coordination. Secondary responsibilities include analyzing and acting on positive Social Determinants of Health (SDOH) screens generated from other services lines, determining when internal Behavioral Health services are needed or external community resources are most appropriate and ensuring follow-through on referrals and outcomes.
ESSENTIAL FUNCTIONS/KEY COMPETENCIES

PRIMARY RESPONSIBILITIES
Behavioral Health Access & Intake Management
- Conducts structured BH intake screenings for new referrals and assigns patients to appropriate psychotherapy or psychiatry panels based on age, diagnosis, and complexity.
- Develops and maintains provider schedules and appointment workflows across clinical sites.
- Coordinates outreach and rescheduling of missed psychotherapy appointments.
- Exercises professional judgment to determine whether patients need integrated care or enabling services.
- Documents all encounters and coordination activities using structured fields, SmartForms, and referral workflows in the Electronic Health Record (EHR).
Integrated Care Coordination & Warm Hand-Offs
- Serves as the coordination point for warm hand-offs between BH, Psychiatry, Medical, Dental, and Enabling Services.
- Determines internal vs. external referral pathways based on clinical indicators or identified social needs.
- Tracks and monitors all assigned referrals, closes the loop on outcomes, and ensures visibility across care teams.
- Communicates patient needs across departments using structured HER workflows to ensure seamless transitions and continuity of care.
- Provides trauma-informed and culturally competent navigation, education, and support throughout patient’s care.
- Coordinates interpreter services, accommodations, and transportation assistance when needed.
- Maintain accurate transportation logs for compliance and reporting.
- Serves as a resource to staff with questions regarding BH access and referral process.
SECONDARY RESPONSIBILITIES
- Reviews positive SDOH screens generated from other service lines to determine the most appropriate next step.
- Independently identifies whether the patient requires BH services, enabling services, or referral to external partners.
- Initiates referrals and completes documentation using structured workflows and SmartForms.
- Tracks referrals and follows up to confirm successful connection to services.
- Collaborates with community organizations and maintains up to date knowledge of available resources, eligibility requirements, and referral pathways.
- Supports compliance, grant, and quality reporting related to SDOH interventions.
- Participates in quality improvement activities that enhance access to behavioral health and integrated care services.
- Reports monthly activity summaries, referral counts, and outcome tracking.
- Identifies workflow gaps or opportunities for improving access, engagement, and care coordination.
- Maintains HIPAA compliance and professional conduct in all communications.
- Other duties as assigned.
KEY DELIVERABLES
Behavioral Health Access & Intake
- Complete BH intake screenings within 5 business days of referral
- Assign psychotherapy patients to panels or waitlist within 2 business days of intake
- Complete outreach and rescheduling of 100% of BH no shows within 48 hours
Integrated Care Coordination
- Facilitate and document warm hand-offs for >90% of eligible patients
- Complete documentation on internal referrals within 30 days
- Document 100% of care coordination using structured SmartForms
SDOH Review & Intervention
- Review 100% of positive SDOH screens from other services lines
- Initiate internal or community referrals within 3 business days of identified need
- Track and confirm completion of all external referrals
- Coordinate transportation and document services within 3 business days
- Maintain transportation logs with zero audit discrepancies
- Submit monthly activity summaries and referral data on time
POSTION REQUIREMENTS
Education/Experience
- Associates Degree in Social Work, Human Services, Psychology, Public Health, or related field required; Bachelor’s degree preferred.
- 2+ years of experience in behavioral health, healthcare, social services or care coordination.
- Knowledge of community resources and referral processes preferred.
- Bilingual (Spanish/English) strongly preferred.
- Strong communication, customer service, and problem solving skills.
- Ability to work independently in a fast paced, team-oriented environment, while maintaining confidentiality and professionalism.
- Ability to sit, stand, and work at a computer workstation for extended periods.
- Frequent computer and telephone use; must be able to communicate clearly in person, over the phone and through electronic systems.
- Occasional movement throughout clinic sites, including exam areas, behavioral health suites, and administrative offices.
- Travel between clinic locations or community agencies.
- Ability to lift and care up to 15-20 lbs. occasionally.
- Work is performed in a standard clinic setting with potential exposure to communicable diseases, bodily fluids, or other health hazards consistent with a healthcare environment.
- Must adhere to infection control protocols, including PPE when required.
- May require occasional evening or weekend hours to support patient needs, community outreach, or organizational events.
- Requires ability to problem solve, prioritize tasks, and perform with multiple interruptions.
WORK SCHEDULE
Open availability to work a minimum of 40 hours during clinic operating hours. Clinics generally operate Monday – Thursday, 7 am – 7 pm, Friday, 7 am – 5 pm, and Saturday, 7 am – 3 pm.
