Job summary

AgeCare in Medicine Hat is seeking a Licensed Practical Nurse (LPN) to join our team. Reporting to the Director of Care, the LPN will deliver patient-centered nursing care in line with provincial standards and collaborate with an interdisciplinary team to support residents' dignity and well-being.

Qualifications

  • Current registration as an LPN in the applicable province.
  • Experience in senior or long-term care preferred.
  • Background in gerontology, rehabilitation, or complex/community care (asset).
  • Strong clinical knowledge and care coordination skills.
  • Current CPR and First Aid certification.
  • Proficiency with electronic documentation and Microsoft Office.
  • Completion of a Vulnerable Sector Check or willingness to obtain.

  • Provide direct nursing care and administer medications and treatments in compliance with policies and regulations.
  • Safely manage medications and narcotics in accordance with scope of practice and legislation.
  • Educate residents and families about health conditions and care plans.
  • Coordinate care plans with physicians, team members, and service providers.
  • Assign responsibilities to the nursing team for efficient service delivery.
  • Respond to significant changes in residents' conditions and manage medical emergencies.
  • Provide direction and delegate tasks to unregulated care providers.
  • Oversee emergency response in the absence of senior leadership.
  • Address resident/family concerns and ensure timely follow-up or escalation.
  • Support infection prevention, health and safety initiatives, and quality improvement plans.
  • Act with integrity and uphold the organization's values, particularly those related to ethics and trust.
  • Perform other related duties as assigned.

The Opportunity

What You Bring
  • Current registration as a Licensed Practical Nurse (LPN) in the applicable province/state.
  • Experience in senior care or long-term care (preferred).
  • Education or background in gerontology, rehabilitation, or complex/community care (asset).
