Job summary

Kensington Health is seeking an Associate Director of Care, Long-term Care for Kensington Gardens. You will lead a team of nurses and care staff, steward budgets, and drive quality, safety and resident experience in a busy LTC home.

Reporting to the Director of Care, Long-term Care, you will implement care plans, ensure regulatory compliance, and foster a just safety culture while advancing equity and inclusion.

Job Overview

Reporting to the Director of Care, Long-term Care, the Associate Director of Care, Long-term Care (LTC), serves Kensington Health's LTC home (i.e., Kensington Gardens) to support clinical operations and advance quality of care, safety, and the experience of the people we serve.

The role is pivotal to ensuring operational excellence by stewarding human, financial and material resources at Kensington Gardens. This role will be guided by the legislation and regulations governing LTC in Ontario, the organization's policies and procedures and best practices for nursing and personal care.

As a key member of the operational leadership team, the Associate Director of Care, Long-term Care, will lead a team of registered nurses, registered practical nurses, personal care attendants and restorative care aids (if applicable) to deliver daily care and services for the people who live the assigned resident home areas (RHAs) at Kensington Gardens.

Responsibilities
Human Resources:
  • Works with human resource team members to effectively recruit andretainregistered nurses, registered practicalnursesand personal care attendants.
  • Works in collaboration with the Health and Safety team to support team members whorequiremodified duties,hoursand/or accommodations in the workplace.
  • Monitors and manages team member performance in collaboration with the Director of Care,LTCand Human Resources to resolve performance issues.
  • Completes regular performance evaluations for registered nurses, registered practical nurses, personal care attendantsand other assigned team members who work in the assigned RHAs.
  • Collaborates with staffing coordinators to ensure all shifts arefilled;addresses unplanned absences and approves any changes to the staff schedule.
  • Reviews and approves/declines time off requests.
  • Supports continuous learning and professional development of team members through education, coaching, and mentorship.
  • Fosters productive relationships with theunionincluding participation inlabourmanagement meetings whenindicated.
  • With guidance from the Director of Care, Long-term Care, fosters a workplace free of violence and harassment and promotes workplace safety bytaking actionwhen concerns are brought forth orobserved.
  • Demonstrates a commitment to equity,diversityand inclusion by championing initiatives that promote equity, diversity, and inclusion.
Financial and Material Management:
  • Provides input via the Director of Care, Long-term Care, into annual operating and capital budgets for LTChome.
  • Makes financial decisions in alignment with their delegated signing authority withgoalof responsible financial stewardship.
  • Ensures clinical equipment and supplies are well-maintained,inventoriedand there are sufficient supplies and equipment at the point of care for the assigned RHAs.
  • Provides input into the minor and major capital equipment needs asrequested.
  • Ensures supplies and materials that are ordered and received arealignto RHA needs and remain within budget.
Clinical Operations:
  • Oversees clinical operations for assigned RHAs with a focus on the care and services provided by registered nurses, registered practicalnursesand personal care attendants.
  • Ensures that the assigned RHAs are consistently staffed within the approved staffing model and budget.
  • Liaises with colleagues on the admissions, transfers and discharges of people living at Kensington Gardens as needed.
  • Ensure the interprofessional care planning process is implemented for each person living at Kensington Gardens including coordination and execution of theinitialand annual care conferences, and as needed, for each person.
  • Monitors the health status of residents in their assigned home areas and liaises with the interdisciplinary team members and resident/substitute decisionmakeras needed.
  • Engages with attendant and consulting physicians and nurse practitioners to foster high quality,timelycare.
  • Ensures that documentation in the health record meets professional and organizational standards for registered nurses, registered practicalnursesand personal care attendants.
  • Ensures that team members are aware of and comply with organizational policies,practices as wellaslegislative and regulatory requirements.
  • Regularly audits the health records of those living at Kensington Gardens with particular focus on residents withhigh riskfactors (e.g.falls, pressure injuries, responsivebehaviours, etc.) andtakes action to addressany care or process gaps.
  • Coordinates the restorative care program for the assigned RHAs, including supervision of the restorative care aides and collaborating with the Managers, Information Systems, for the purpose of coding in the Resident Assessment Instrument/Minimum Data Set (RAI/MDS) clinical assessments.
  • Leads the implementation, monitoring and annual evaluation of a clinical program that drives quality of care and improvement in outcomes as assigned (e.g.skin and wound, falls,continencecare, pain management, medication safety, etc.).
Quality, Safety and Experience:
  • Fosters a service excellence culture through role modeling the values of the organization.
  • Oversees and supports the resolution of inquiries from the people who live in the LTC home and their loved ones, including following the complaints escalations policy and fulfilling any notification and disclosure obligations asrequired.
  • Supports the achievement of quality improvement goals andobjectives(e.g.annual quality improvement plans) by leading and/or supporting the implementation of initiatives.
  • Promotes a just safety culture through timely reporting (internal and external), completion of safety incident reviews and sharing of system and process learnings with their team.
  • Promotes person-centred approaches to care and service through role modeling and fostering relationships with the Residents and Family Councils.
Other:
  • Participates in committees and engagement forums as required by the organization.

Required Knowledge and Experience:
  • Minimum 5 years of formal management roles in healthcare operations; Long-term Care sector preferred.
  • Has critical assessment skills that aide in constructive problem solving and influencing and resolving conflict.
  • Demonstrated leadership in implementing initiatives related to safety and quality improvement.
  • Proventrack recordofsuccessimplementation of change initiatives.
  • In depth experience related to clinical assessment, planning and interventions related to the care of people with complex needs, and particularly those living in a long-term care setting.
  • Knowledge of infection prevention and control principles and practices.
  • Knowledge of applicable legislation and regulations (e.g., Occupational Health and Safety Act, Employment Standards Act, Fixing Long-term Care Act, Health Care Consent Act, Personal Health Information Protection Act (PHIPA) etc.).
  • Upholds strict compliance to reviewingandreleasingconfidential documents, especially those that include personal health information. Follows release of information processes that align with corporate policy and PHIPA legislation.
Required Professional Designation/Certificate:
  • Bachelor's degree in a Regulated Health Professional Field.
  • Master's degree preferred (e.g., nursing, health administration, leadership).
  • Active member in good standing with the Regulated College of Ontario.
  • Certificate in Gentle Persuasive Approaches, Non-Violent Crisis Intervention and/or change management preferred.
