Job summary

Everyday Independence is seeking a Youth Lead Practitioner to support adolescents and young adults with disabilities. You will help plan NDIS funded therapy packages and work across homes, schools and community settings to achieve education, personal, and social goals.

Qualifications

• Minimum 2 years experience working with youth aged 9-25.
• Relevant tertiary qualification and registration required.
• Australian driver’s license and compliance checks are required.

Responsibilities

• Lead the interdisciplinary youth team planning a young person’s NDIS funded therapy package.
• Support youth participants to achieve educational, personal, social, and working life goals.
• Deliver therapy in diverse settings including homes, schools, playgrounds, workplaces and community centres.
• Document and bill each session through the online participant management system.
• Attend weekly team meetings with the Hub, and engage with your Team Leader and Therapy Mentor.

Location – Newtown

Employment Type – Full time, Part time (4 days per week) or 9-day fortnight

Salary – $80,104.00 - $93,120.80 per annum (inclusive of annual leave loading) + superannuation + vehicle options + mobile + laptop + options to boost salary up to $10,000

Supervision and Support – from supervisors, team leaders and peers

About The Role

We are looking for Youth Lead Practitioners to support adolescents and young adults aged 9-25 as they navigate life with a disability, from adolescence through to early adulthood.

As a Youth Lead Practitioner At Everyday Independence, No Two Days Will Be The Same. If You Were Here Last Week, These Are Some Of The Things You May Have Done

• Led the interdisciplinary youth team that plans a young person’s NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families
• Utilised assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working life goals
• Worked within the social model which means you’ll have worked and delivered therapy in diverse settings, from homes and schools to playgrounds, workplaces and community centres
• Held meaningful, future-focused conversations with participants and their supports
• Empowered participants and their families to develop habits and routines
• Built meaningful relationships with your participants and their supports (family, carers, friends)
• Documented, and billed for, each session through our online participant management system
• Attended the weekly team meeting in the Hub, sharing stories of impact, coffee and laughs
• You’ll have met with your Team Leader and Therapy Mentor to tap into their clinical knowledge and/or support
