Job summary

Activate Community Services – Mornington VIC is seeking a compassionate Registered Nurse to deliver community nursing services across Mornington Peninsula and nearby suburbs. Role is casual/part-time with home visits and independent practice.

Responsibilities include nursing assessments, care planning, wound care, and coordination with GPs and families. Current AHPRA registration and relevant community/home-care experience are preferred.

• Current registration with AHPRA as a Registered Nurse.
• Experience in community nursing, home-based care, or wound management is desirable.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently and manage community visits.

Position: Registered Nurse – Community Nursing

Key Responsibilities

Deliver community nursing services to eligible clients in their homes and community settings.

Conduct nursing assessments and monitor clients’ health conditions.

Develop, implement, and review individualized nursing care plans.

Provide wound care, medication support, catheter care, and other nursing interventions within your scope of practice and authorized service arrangements.

Maintain accurate clinical documentation and nursing records.

Communicate and collaborate with general practitioners, allied health professionals, families, and other relevant stakeholders.

Ensure all services are delivered in accordance with professional nursing standards and organizational policies and procedures.

Promote client dignity, independence, choice, safety, and quality of life through compassionate and respectful care.

Identify and report changes in clients’ health conditions, clinical risks, and incidents, taking appropriate action when required.

Provide health education and support clients to manage their health conditions and maintain independence in their own homes.

Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse.

Relevant nursing qualifications and clinical experience.

Experience in community nursing, home-based care, or wound management is highly desirable.

Strong clinical assessment, communication, and documentation skills.

Ability to work independently and manage community visits effectively.

Current First Aid and CPR certification, as required.
