Job summary

Colac Area Health invites applications for the Barwon Orange Door - Colac Access Point Practitioner. The role focuses on integrated intake for child wellbeing and family violence, with screening, triage, and planning responsibilities.

The successful candidate will work across Colac Access Point and Barwon Orange Door sites, applying trauma-informed approaches and collaborating with a multidisciplinary team.

Qualifications

• Bachelor of Social Work or equivalent qualification, or willingness to work towards minimum qualification.
• Experience with families/children with complex needs, confidentiality and safety planning.
• Ability to work with diverse socio-economic and cultural backgrounds.
• Multidisciplinary team collaboration and integrated practice.

Responsibilities

• Provide screening, identification, triage, assessment and planning for child wellbeing and family violence referrals.
• Apply trauma-informed practice to promote client choice and empowerment.
• Collaborate with partner agencies within an integrated practice framework.
