An Aboriginal health service provider is seeking a Team Leader for the Heart Health Program in Perth. This part-time role will focus on improving cardiovascular health in the Aboriginal community through health education and support. Derbarl Yerrigan Health Service is looking for a committed Aboriginal Health Practitioner, Registered Nurse, or Enrolled Nurse to strengthen the heart health of Aboriginal community members. You’ll provide culturally safe care while empowering participants with health education and meaningful community support.
The Derbarl Yerrigan Health Service (DYHS) was established in 1974 and is the largest Aboriginal community-controlled health service provider in Western Australia, providing care to over 20,000 Aboriginal patients across four clinics in East Perth, Mirrabooka, Maddington and Midland. Our staff enable the delivery of culturally responsive primary health services, focusing on prevention, early intervention, comprehensive care and care coordination including chronic disease management.

The Team Leader will ensure that the program’s initiatives are implemented effectively, with a focus on empowering participants through health education and community support. This part-time role (0.4 FTE) is crucial to the coordination of Derbarl Heart Health Program, which aims to improve the cardiovascular health and well-being of Aboriginal people through graduated exercise, dietary interventions, and social support.
This will be a Maximum term Part-time contract which expires on 30 June 2027, but with possibility of extension.
