Qualifications

Derbarl Yerrigan Health Service is looking for a committed Aboriginal Health Practitioner, Registered Nurse, or Enrolled Nurse to strengthen the heart health of Aboriginal community members. You’ll provide culturally safe care while empowering participants with health education and meaningful community support.

About the Organisation

The Derbarl Yerrigan Health Service (DYHS) was established in 1974 and is the largest Aboriginal community-controlled health service provider in Western Australia, providing care to over 20,000 Aboriginal patients across four clinics in East Perth, Mirrabooka, Maddington and Midland. Our staff enable the delivery of culturally responsive primary health services, focusing on prevention, early intervention, comprehensive care and care coordination including chronic disease management.

About the Position

The Team Leader will ensure that the program’s initiatives are implemented effectively, with a focus on empowering participants through health education and community support. This part-time role (0.4 FTE) is crucial to the coordination of Derbarl Heart Health Program, which aims to improve the cardiovascular health and well-being of Aboriginal people through graduated exercise, dietary interventions, and social support.

This will be a Maximum term Part-time contract which expires on 30 June 2027, but with possibility of extension.

Key Responsibilities
  • Work with external exercise physiologists, cardiac rehab nurses and internal staff to ensure the smooth coordination of weekly heart health sessions.
  • Delegate a driver, ensure patients are booked in for assessments, and work with the Nurse practitioner to ensure all patients participating in the project have up to date care plans and team care arrangements.
  • Provide guidance and support to Aboriginal Health Practitioners, Nurses, General Practitioners, and students in the clinic.
  • Ordering of equipment such as spill kits, sharp container, medical supplies, vaccines, oxygen cylinder and pathology supplies for the heart health program.
  • Organise catering, transport, and external speakers for the heart health program.
Selection Criteria
  • Aboriginal and/or Torres Strait Islander pursuant to Section 50D of the Equal Opportunity Act.
  • Must be a qualified Aboriginal Health Practitioner, Registered Nurse or Enrolled Nurse.
  • Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Demonstrated experience in coordinating, supervising, and supporting a team, and working in a multi-disciplinary team environment.
  • Knowledge of current health issues, impacting on Aboriginal Health.
  • Demonstrated ability to maintain confidentiality and security of records and Information.
  • Well-developed written and verbal communication skills.
  • Knowledge of Work Health & Safety, Industrial Relations, and Equal Employment Opportunity Legislation.
Appointment Pre‑requisites
  • Certificate IV or Diploma in Aboriginal Health, or equivalent Nursing qualifications.
  • Valid Western Australia Drivers Licence
  • Current Senior First Aid Certificate
  • Mandatory Covid 19 Vaccinations
  • Satisfactory National Police Clearance, which is less than 6 months old
  • Successful Pre-Employment Health Assessment
