Transform Life Pty is seeking an experienced Behaviour Support Practitioner to lead and grow our Western Australian team. The role blends clinical leadership, practitioner development, and service operations, with a small local caseload to keep you connected to practice.

You’ll mentor and supervise practitioners, guide complex cases, onboard new team members and help shape PBS delivery across WA. A proactive, collaborative leader will thrive here.

Qualifications

• NDIS registration at Proficient/Advanced/Specialist level.
• Experience mentoring, supervising or leading others.
• Operational coaching skills to drive improvements.
• Relevant tertiary qualifications and experience in disability or allied health.

Responsibilities

• Coach and support team members through planning and workload management.
• Mentor and supervise practitioners to build confidence and clinical capability.
• Guide complex cases and clinical decision-making.
• Support onboarding and development of new team members.
• Identify practice gaps, set expectations and address challenges constructively.

Transform Life is looking for an experienced Behaviour Support Practitioner to help lead and develop our Western Australian team.

This role combines clinical leadership, practitioner development and service operations, with a small local caseload to keep you connected to practice.

You’ll have the opportunity to shape how the team works, support practitioners through complex cases and contribute ideas as our WA service continues to grow.

You’ll be joining an established team of eight practitioners, supported by experienced Senior PBS Leaders, a Reports Review team and our National PBS Manager, who is based in WA.

What you’ll bring

You’re a confident, approachable practitioner who enjoys helping others develop. You can work independently while staying connected to your team, navigate complex situations calmly and balance support with accountability.

You’ll also bring:

• NDIS Quality and Safeguards Commission registration as a Proficient, Advanced or Specialist Behaviour Support Practitioner.
• Experience mentoring, supervising or leading others.
• Operational skills and ability to coach others on improvements
• Relevant tertiary qualifications and experience in disability, allied health, mental health, education, social work, or a related field.
