Job summary

Mackay Base Hospital in Mackay is seeking a Staff Specialist, Rehabilitation to join the Medicine Department on a Permanent Full-Time basis.

The role involves providing specialist medical assessment, diagnosis and ongoing management for rehabilitation patients, with clinical oversight of the rehabilitation service and collaboration with a multidisciplinary team.

Responsibilities

• Provide specialist medical assessment, diagnosis, treatment and ongoing management of patients requiring rehabilitation.
• Provide clinical oversight of patients admitted to the rehabilitation service, including regular review and management of complex medical and rehabilitation needs.
• Develop and review individualised rehabilitation and management plans in collaboration with patients, carers and the multidisciplinary team.
• Participate in multidisciplinary case conferences, ward rounds and clinical meetings.
• Work collaboratively with allied health, nursing, medical and other clinical staff to facilitate coordinated patient care.
• Support timely and appropriate discharge planning, including liaison with community services, primary care providers and other health services.
• Provide appropriate medical management of patients with complex comorbidities and rehabilitation needs.
• Provide consultation and advice to other medical teams regarding patients requiring rehabilitation input.
• Contribute to outpatient rehabilitation clinical and follow up services as required.
• Maintain appropriate clinical documentation, including timely completion of discharge summaries and correspondence
