Job description
About the role

This Social Work position is based within the Transplant Services in the Heart and Lung team at The Prince Charles Hospital. This position comprises 0.6 FTE permanent funding, with an additional 0.4 FTE provided as temporary funding for a 12-month period.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values :

  • Deliver quality Social Work services in accordance with the Australian Association of Social Workers (AASW) Code of Ethics and AASW Professional Practice Standards to optimise the psychosocial outcomes, and to achieve maximum benefits from health care services to patients and their families / carers.
  • Work collaboratively within a multidisciplinary team / s by effectively providing Social Work leadership to achieve optimal health outcomes for patients, families and caregivers.
  • Contribute to the strategic direction of Transplant Services and Social Work Service through participation in the team meetings and other service development initiatives and quality activities.
  • Relevant to the position, participate in the ongoing education, implementation, monitoring and evaluation of safety and quality initiatives relevant to The National Safety and Quality Health Services Standards and the Australian Council on Healthcare Standards (ACHS) Evaluation and Quality Improvement Program (EQuIP).
