As a Social Worker with Guide Healthcare, you’ll support older adults to navigate life changes, maintain, and stay connected to their community. This role focuses on practical, person-centred support for clients experiencing cognitive change, anxiety, adjustment to ageing, or loss of independence. You’ll work alongside allied health teams to help clients build coping strategies, maintain routines, and stay engaged with their community.
