Qualifications

  • Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
  • Ability to demonstrate practical application of the AASW Code of Ethics.
  • Understanding ageing, age-related illnesses, rehabilitation, and end-of-life care.

Responsibilities

  • Provide high quality social work services to consumers, carers and families to ensure optimal health outcomes.
  • Assist with assessment, counselling, crisis management, case management and appropriate referrals.
  • Advocate for services and resources in line with NSLHD policies and NSW Health standards.

Job description

We are seeking a passionate Social Worker to join our Community Aged Care team on a part-time basis. This is an opportunity to deliver person-centred care, build strong community connections, and positively impact the lives of older adults.


Employment Type: Temporary Part Time until 6 February 2028 (with possibility of ongoing employment)


Position Classification: Social Worker Level 1/2


Remuneration: $40.05 - $57.14 per hour


Hours Per Week: 24

What You'll Be Doing


Provide a high quality Social Work service to consumers, carers and families to ensure the provision of optimal health outcomes consistent with NSLHD policies, procedures, standards, relevant legislation, and NSW Health Code of Conduct.


People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.


Candidates Will Need To Meet The Following Criteria



  • Social Work degree and eligibility for the Australian Association of Social Workers (AASW), including the ability to demonstrate a practical application of the AASW Code of Ethics.

  • Demonstrated understanding of issues associated with ageing, age related illnesses, rehabilitation, accommodation placements, guardianship and end of life care.

  • Demonstrate skills and knowledge in the provision of service to consumers, carers and families requiring assessment, counselling, crisis management, case management, advocacy, intervention, appropriate referral.
