Job summary

Mercy Health in Werribee invites a Social Work / Care Coordination professional to join the Health Independence Program team. Based in a dynamic office, you will support clients in the community and outpatient settings with assessments, crisis intervention, and referrals.

The role requires a Bachelor or Masters of Social Work, eligibility for ASW, and police/WWCC checks. Full Victorian Driver’s Licence and experience in advocacy, mental health, housing or elder care are highly regarded.

Qualifications

  • Social Work degree required (Bachelor or Masters) and eligibility for ASW.
  • Experience in community health, mental health, housing, carers' needs.
  • Current Police Check and Working with Children Check required.
  • Full Victorian Driver’s License (no restrictions)

Responsibilities

  • Home visits and comprehensive assessments in community/outpatient settings.
  • Provide psychosocial assessments and crisis intervention as needed.
  • Assist clients to access services (Centrelink, NDIS, VCAT) and coordinate referrals.
  • Present at case conferences and facilitate family meetings.
  • Contribute to interdisciplinary team care planning and care coordination.

About the opportunity:

As part of HIP you will be based in a dynamic office working alongside a multidisciplinary team of Allied Health Staff and Specialist Nurses who provide a high level of support and care coordination to HIP clients within the Wyndham catchment.

This is a high performing team working Monday to Friday, office hours. You are not required to work weekends or public holidays.

HIP consists of 4 services; Community Based Rehabilitation and Complex Care Coordination; as well as Falls and Balance, and Continence Clinic .

The Social Work /Care Coordination role services Complex Care Coordination as a Care Coordinator and the Community Based Rehab Team in a Social Work capacity.

  • Supporting clients in the community and within the outpatient setting, your role would include: home visits, comprehensive assessments, comprehensive psychosocial assessments, crisis intervention, supporting clients to access services such as Centrelink, NDIS or VCAT, facilitating family meetings, completing appropriate referrals and presenting at case conferences.
  • The position requires an extensive understanding and awareness of working in a community setting - community experience is desirable.
  • The successful applicant will demonstrate ability to complete complex care need assessments, with extensive knowledge and experience in applying the most appropriate interventions to best support the client, including for mental health issues.
  • Utilising therapeutic counselling skills
  • Utilising motivational interviewing techniques to help clients explore and resolve their uncertainty, as well as setting SMART goals

You will also be required to provide evidence of, or in the process of obtaining:

  • A current Police Record Check and Working with Children Check.
  • Proof of current influenza vaccination and other vaccination status consistent with public health orders.
  • Full Victorian Driver’s License (no restrictions)
  • Broad range of experience in areas such as advocacy, mental health, housing and homelessness, carer related needs, elder abuse or psychosocial issues
  • Mandatory Bachelor or Masters of Social Work
  • Eligibility for membership with the Australian Association of Social workers
