Job summary

Darling Downs Health in Warwick is seeking a Social Worker to deliver person-centred support across hospital and community settings, helping individuals and families access care at the right time.

You'll work autonomously within a connected multidisciplinary team, build strong relationships with patients and families, mentor emerging practitioners, and advance regional practice through collaboration, advocacy and continuous learning.

• Strong clinical assessment, intervention and case management skills.
• Ability to work effectively within multidisciplinary teams and build positive stakeholder relationships.
• A current C class driver’s licence and willingness to travel.
